Attached is the 2018 VPP Annual Self-Evaluation form (2018 VPP participant self eval final.20181121 FINAL.doc).
Because of the lateness in distributing these documents, the OSHA National Office is adding a 30-day extension this year. The complete responses for the annual self-evaluations will be due by Friday March 15, 2019. Please submit to both Mr. Richard Brown (brown.richard@dol.gov) and Ms. Greta Olsson (olsson.greta@dol.gov) via email prior to the deadline.
Only our 11 Region 2 VPP sites with a PSM-covered process need to complete a 2018 PSM Supplement B. This year, the PSM Supplement B is in two parts – a document (2018 Supp B Final.20181107.docx) and a spreadsheet (2018 PSM Supplement B Worksheet.20181108.xlsx). The completed responses for the PSM Supplement B as well as completed 2018 VPP self-evaluation are both due by Friday March 15, 2019. If your VPP site has a PSM-covered process, please submit these documents in a single email to both Mr. Richard Brown and Ms. Greta Olsson.
Attached is the 2017 BLS Injury Rate Chart (2017 BLS Data.xlsx). Please use this data for comparing your site’s three-year TCIR and DART rates.
