2017 VPP Self-Assessment Template
Here are the 2017 VPP Annual Self-Evaluation template and related attachments, for reference during tomorrow's webinar.
"Attached is the 2017 VPP Annual Self-Evaluation template that every VPP site must complete and submit by Thursday February 15, 2018. Please note that there are minor changes from last year's template in sections G, H & I. Please submit to both myself (brown.richard@dol.gov) and Ms. Greta Olsson (olsson.greta@dol.gov) via email prior to the deadline. (DOC file attached)
The US Bureau of Labor Statistics (BLS) released the 2016 Injury and Illness Rates earlier today. You will need the 2016 BLS data to compare your site's TCIR and DART rates against your industry's averages. A link is provided below, and attached is a XLSX file with the data.
Our VPP sites with a PSM-covered process must also complete a 2017 PSM Supplement B. (DOC file attached) The completed responses for the PSM Supplement B as well as completed annual self-evaluations are both due by February 15, 2018. If your VPP site has a PSM-covered process, please submit both documents in a single email to both myself and Ms. Greta Olsson.
PLEASE NOTE: As we have done in the past, we will conduct a FREE webinar for our Region 2 VPP participants to review these specific forms. We'll also go over Best Practices for performing a VPP self-evaluation. The webinar has been scheduled for Wednesday January 3, 2018 at 10:00 and will last about an hour. I will email the login information to all of our Region 2 VPP Site Contacts as we get closer to that date. In the meantime, please feel free to contact me with any questions.
